Sign in Sheet Instructions
Drivers will be required, by the delivery vendor who employs them, to initial the sheet and provide the date and time of the stop. After the stop but on the same day, library staff will initial the form and provide the date and time. The library staff's initials, etc. assure that delivery site is checked in a timely manner.
- Library staff are not required to be at the pick-up site when the driver comes.
- Staff are required to leave the materials in the same location every day and have the materials ready for the driver by the usual stop time.
- The sign-in sheet must be in the same location every day and be as close to the LORI bins as possible.
- Sign-in sheets are sent to OLIS ONLY if there is a delivery problem that requires a record of a stop. Otherwise, they can be destroyed two months after the last date on a sheet. this will give us ample time to clear our billing for the previous month.
The purpose of the sign-in sheet is to provide information that OLIS needs to resolve LORI member delivery problems. The delivery vendor fully supports the procedure.
Please print a copy of the form and use as your library's master copy. Make photocopies of it to use at your delivery location. A copy of the form and of this memo were also sent out in the delivery service on Wednesday, January 24, 2001.
