Submission of LORI grant application

Submit: A copy of the completed grant application by email to: donnadm@gw.doa.state.ri.us on or before June 20, 2006.
In addition, submit the signed signature page by fax no later than June 20 or by U.S. Mail (postmarked no later than June 20):

Each application should include the following parts:

• Part 1 Application Cover Page
• Part 2 Proposal
• Project Narrative: Fuller description and questions to assist in forming the proposal can be found with the LORI Grant Application. The sections of Part 2, Proposal are:
• Goals and Description
• Plan of Operation
• Monitoring and Evaluation
• Reports
• Part 3 Budget and Cost Effectiveness

Selection Criteria

The maximum score for each proposal is 100 points with each criterion assigned a maximum number of points as follows:

• Project Goals and Description   = 20
• Plan of Operation   = 20
• Budget and Cost-Effectiveness   = 15
• Monitoring and Evaluation of Project   = 30
• Appropriateness: Degree to which project speaks to the purpose and priorities of the LORI Grant program   = 15
   
• Total   = 100

A LORI Working Group will be appointed from LORI libraries not affiliated with libraries applying for LORI grants in the current cycle. The Working Group will be made up from staff at various types of libraries so as to represent a multitype system. The Working Group will have two weeks to review the applications. The Working Group will score the applications according to the criteria listed above, review and discuss applications and total scores, and make recommendations to OLIS for funding.
